Эндопротезирование тазобедренного сустава (ЭТБС) является эффективным методом лечения при поздних стадиях поражения сустава у пациентов с ревматоидным артритом (РА), обеспечивающим уменьшение болевого синдрома и улучшение функции. Системное воспаление, вторичный остеопороз и особенности противоревматической терапии осложняют хирургическое лечение данных больных. Изучение взаимосвязи вариантов базисной терапии имеет большое значение для оптимизации периоперационного лечения и улучшения клинических результатов ЭТБС.</p><p>Цель работы — оценить влияние различных вариантов базисной противоревматической терапии на послеоперационные исходы эндопротезирования тазобедренного сустава у пациентов с ревматоидным артритом.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведён анализ 314 клинических случаев первичного тотального ЭТБС. В исследование включены пациенты с РА (n = 194) и инволютивным артрозом (ИА) (n = 120). Оценивали рентгенологические изменения (классификации Келлгрена – Лоуренса и Paprosky, кортикальный индекс), особенности оперативного лечения, объём интраоперационной кровопотери, исходы операции, частоту осложнений, динамику боли по визуально-аналоговой шкале, функцию сустава по шкале Harris Hip Score, активность РА по индексу DAS28 и качество жизни по шкале HAQ.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов с РА выявлены более выраженные ацетабулярные дефекты и снижение кортикального индекса по сравнению с больными ИА (p &lt; 0,0001). Применение базисных противовоспалительных препаратов ассоциировалось с более сохранной костной структурой, тогда как длительная терапия глюкокортикостероидами сопровождалась ухудшением рентгенологических показателей и увеличением кровопотери. Интраоперационная кровопотеря была выше при РА, особенно на фоне приема глюкокортикостероидов и нестероидных противовоспалительных препаратов. Через три месяца после ЭТБС в обеих группах отмечено улучшение клинических результатов, снижение боли и улучшение функции сустава, однако у пациентов с ИА достигнуты более значимые результаты.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Полученные данные подтверждают ключевую роль системного воспаления и характера противоревматической терапии в формировании хирургических рисков и функциональных исходов ЭТБС. Также важно влияние вида противоревматической терапии на интраоперационные особенности ЭТБС и исходы оперативного лечения.</p></sec><sec><title>Заключение</title><p>Заключение. ЭТБС — метод оперативного лечения, который способен улучшить клинико-функциональные исходы и повысить качество жизни пациентов с РА. Длительная стероидная терапия пагубно влияет на состояние костной ткани, осложняет проведение операции и увеличивает объём интраоперационной кровопотери.</p></sec></abstract><trans-abstract xml:lang="en"><p>Introduction Total hip arthroplasty (THA) is an effective treatment for end-stage hip joint involvement in patients with rheumatoid arthritis (RA), reducing pain and improving function. However, systemic inflammation, secondary osteoporosis, and the specific nature of antirheumatic therapy can complicate surgical treatment of the patients. Understanding the interrelationships between standard therapy options is crucial for optimizing perioperative management and improving clinical outcomes of THA.</p><p>The objective was to evaluate the impact of different variants of disease-modifying anti-rheumatic therapy on postoperative outcomes of THA patients with rheumatoid arthritis.</p><p>Material and methods A review of 314 clinical cases of primary THA was performed. The study included patients with RA (n = 194) and involutional arthrosis (IA) (n = 120). The parameters assessed included radiographic changes (Kellgren-Lawrence and Paprosky classifications, cortical index), surgical treatment, intraoperative blood loss, surgical outcomes, complication rate, pain dynamics evaluated with VAS, joint function assessed with the Harris Hip Score, RA severity assessment using the DAS28, and the quality of life (QoL) using the HAQ questionnaire.</p><p>Results RA patients were characterized by more pronounced acetabular defects and a reduced cortical index compared to IA patients (p &lt; 0.0001). The use of disease-modifying antirheumatic drugs was associated with better bone structure preservation, while long-term glucocorticosteroid therapy was associated with worsening radiographic parameters and greater blood loss. Intraoperative blood loss was higher in RA with the use of glucocorticosteroids and nonsteroidal anti-inflammatory drugs. Both groups showed improved clinical outcomes, decreased pain, and improved joint function at three months of THA; however, IA patients achieved more significant results.</p><p>Discussion The findings suggested the key role of systemic inflammation and the nature of antirheumatic therapy in determining surgical risks and functional outcomes of endovascular knee replacement. The type of antirheumatic therapy effecting intraoperative features of endovascular knee replacement and surgical outcomes were also highlighted.</p><p>Conclusion THA is a surgical treatment modality that can improve clinical, functional outcomes and quality of life of RA patients.
