Термин «стенки вертлужной впадины» встречается в публикациях по анализу выживаемости эндопротезов и в различных классификациях операций по замене тазобедренного сустава. Несмотря на накопленный опыт эндопротезирования тазобедренного сустава, в публикациях нет информации по определению границ стенок вертлужной впадины. В настоящее время разработана оригинальная классификация посттравматических деформаций вертлужной впадины ASPID, но в ней не  описан способ определения границ стенок.Цель работы — продемонстрировать и теоретически обосновать методику определения границ стенок вертлужной впадины в рамках первичного эндопротезирования тазобедренного сустава.Материалы и методы. Использованы данные компьютерной томографии костей таза пяти детей  10–12 лет и 30 препаратов костей таза взрослых людей без признаков дисплазии вертлужной впадины.Результаты. При анализе 3D-моделей компьютерных томограмм костей таза детей определены экстраацетабулярные фиксированные анатомические ориентиры, через которые можно провести плоскости, разделяющие детскую вертлужную впадину на условные стенки, разделённые хрящом. Выполнено сканирование костей таза 30 взрослых людей и проведены аналогичные построения, позволяющие провести границы стенок вертлужной впадины. Определены долевое соотношение и площади каждой стенки вертлужной впадины в группах детей и взрослых, осуществлено сравнение полученных результатов. Отсутствие статистических различий в группах детей и взрослых при определении долей верхней, задней и медиальной стенок вертлужной впадины доказывает высокую достоверность данной методики.Обсуждение. Статистические различия между группой детей и взрослых в определении доли передней стенки вертлужной впадины незначительны и, вероятно, связаны с небольшим количеством измерений в группах. Отсутствие статистических различий в группах детей и взрослых при определении долей верхней, задней и медиальной стенок вертлужной впадины подтверждает достоверность методики. Для дальнейшей оценки достоверности данной методики требуется большее количество измерений, выполненных несколькими специалистами, с определением коэффициента Каппа Коэна, и статистический анализ результатов.Заключение. Предложенная методика определения границ стенок вертлужной впадины актуальна в научных исследованиях, но не применима в рутинной практике в связи со сложностью и необходимостью использования специального программного обеспечения. Способ рекомендован для описания посттравматических деформаций стенок вертлужной впадины только в случае, когда экстраацетабулярные ориентиры не повреждены и не смещены.</p></abstract><trans-abstract xml:lang="en"><p>Introduction The term "acetabular walls" is used to describe the survival rate of total hip replacements (THR) and classifications of THR surgeries. With the experience accumulated in THR surgery, there is no  information  on  establishing boundaries of the acetabular walls. ASPID is a classification system used to describe post‑traumatic acetabular deformities with no technique for establishing boundaries of acetabular walls.The objective was to demonstrate and provide a theoretical substantiation for a method establishing the boundaries of the acetabular walls in primary THR.Material and methods Pelvic computed tomography scans of five children aged 10–12 years and 30 pelvic preparations of adult bones without signs of acetabular dysplasia were used.Results Extraacetabular fixed anatomical landmarks were identified in 3D models from CT scans of pediatric pelvic bones with the planes dividing acetabulum into conditional walls separated by cartilage. The pelvic bones of 30 adults were scanned, and similar reconstructions performed to delineate boundaries of the acetabular walls. The proportional ratios and areas of each acetabular wall were determined in the pediatric and adult groups, and the results compared. The absence of statistical differences in the proportions of the superior, posterior and medial walls of the pediatric and adult acetabulum suggested high reliability of the method.Discussion There were insignificant statistical differences in the anterior acetabular wall fraction of children and  adults and could be associated with a small quantity of measurements. A larger number of measurements to be performed by several specialists, determination of the Cohen's Kappa coefficient and statistical analysis of the results are essential for the validity of the method.Conclusion The method offered for establishing boundaries of the acetabular walls can be practical for scientific research, but cannot be used in routine practice due to its complexity and the need to use special software.
