Надмыщелковые переломы плечевой кости — наиболее распространенные переломы локтевой кости у детей, часто возникающие в результате падения на вытянутую руку. Стандартное лечение пострадавших с переломами со смещением включает закрытое вправление и чрескожное штифтование. Однако оптимальная конфигурация штифтов, перекрестная (медиально-латеральная) или параллельная (латерально-латеральная), остается предметом дискуссий из-за опасений по поводу стабильности и риска ятрогенного повреждения локтевого нерва.Цель работы — сравнить клинические и рентгенологические результаты применения перекрестного и  параллельного штифтования при лечении детей с надмыщелковыми переломами плечевой кости со смещением.Материалы и методы. Проспективное обсервационное исследование проводили в течение 18 месяцев в государственном медицинском колледже Kalpana Chawla (Карнал, штат Харьяна). В исследование включено 54 ребенка в возрасте от 3 до 12 лет с надмыщелковыми переломами плечевой кости III типа по Гартланду. Пациенты разделены на две группы в зависимости от хирургической техники: перекрестное (n = 27) или параллельное (n = 27) штифтование. Обе группы были сопоставимы по демографическим показателям, механизму травмы и предоперационному нейрососудистому статусу. Функциональные и рентгенографические результаты оценивали по критериям Флинна, углу Баумана, углу переноса и амплитуде движений в интервалах наблюдения 3, 6, 10, 14 и 24 недель.Результаты. Средние значения угла Баумана, угла переноса и диапазона движения не имели статистически значимых различий между группами. На заключительном этапе наблюдения 92,6 % пациентов в группе параллельного штифтования имели отличные результаты по критериям Флинна по сравнению с 51,9 % в группе перекрестного штифтования (p &lt; 0,01). У одного пациента в группе перекрестного штифтования развилась нейропраксия локтевого нерва, в то время как в группе параллельного штифтования случаев повреждения нервов не зарегистрировано.Заключение. Параллельное штифтование продемонстрировало лучшие рентгенологические и функциональные результаты, а также более низкий риск повреждения локтевого нерва по сравнению с перекрестным штифтованием. Полученные данные свидетельствуют о том, что параллельное штифтование должно быть предпочтительным методом стабилизации надмыщелковых переломов плечевой кости со смещением у детей.</p></abstract><trans-abstract xml:lang="en"><p>Introduction Supracondylar humerus fractures are the most common elbow fractures in children, often resulting from falls on an outstretched hand. The standard treatment for displaced fractures involves closed reduction and percutaneous pinning. However, the optimal pin configuration — cross-pinning (medial‑lateral) versus parallel pinning (lateral-lateral) — remains a topic of debate due to concerns regarding stability and risk of iatrogenic ulnar nerve injury.Aims This study aims to compare the clinical and radiological outcomes of cross-pinning versus parallel inning in the management of displaced supracondylar humerus fractures in children.Methods A prospective observational study was conducted over 18 months at Kalpana Chawla Govt. Medical College, Karnal, Haryana. A total of 54 children aged 3–12 years with Gartland type III supracondylar humerus fractures were enrolled. Patients were divided into two groups based on the surgical technique: cross-pinning (n = 27) and parallel pinning (n = 27). Both groups were comparable in terms of demographics, mechanism of  injury, and pre-operative neurovascular status. Functional and radiographic outcomes were evaluated using Flynn’s criteria, Baumann’s angle, carrying angle, and range of motion at follow-up intervals (3, 6, 10, 14, and 24 weeks).Results Mean Baumann’s angle, Carrying angle and range of motion showed no statistically significant differences between the two groups. At the final follow-up, 92.6 % of patients in the parallel pinning group had excellent outcomes per Flynn’s criteria, compared to 51.9 % in the cross-pinning group (p &lt; 0.01). One patient in the cross-pinning group developed ulnar nerve neuropraxia, whereas no cases of nerve injury were reported in the parallel pinning group.Conclusion Parallel pinning demonstrated superior radiological and functional outcomes, with a lower risk of ulnar nerve injury compared to cross pinning.
