Высокий риск инфекционных осложнений при огнестрельных ранениях остается актуальной проблемой военной медицины. Анализ структуры санитарных потерь показывает, что ранения конечностей составляют от 55 % до 81,4 %, при этом около 35 % из них сопровождаются переломами костей. Выполнение операций по окончательной стабилизации данных переломов в условиях первичной микробной контаминации ассоциируется с высоким риском развития инфекционных осложнений. Однако использование антибактериальных покрытий на внутренних имплантатах значительно снижает риски развития таких осложнений.</p><p>Цель работы — на основании анализа российских и зарубежных литературных источников определить эффективность использования антибактериальных покрытий титановых имплантатов при огнестрельных переломах.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Поиск научных публикации осуществляли в поисковых системах eLibrary, PubMed и Connected Papers по ключевым словам: antibacterial coatings, gunshot fractures, implantassociated infection, internal osteosynthesis, infectious complications, антибактериальное покрытие, огнестрельные переломы, инфекционные осложнения, периимплантная инфекция. Источники отобраны исходя из гипотезы о возможности применения антибактериальных покрытий в клинической практике. Глубина поиска — с 2009 по 2025 гг.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Существующие системы доставки антибактериальных препаратов в зону проведения оперативного вмешательства демонстрируют высокую клиническую эффективность в профилактике периимплантной инфекции. На сегодняшний день наиболее изученными агентами для создания покрытий являются ионы металлов, полимеры, а также композиты, содержащие в своем составе антибактериальные/антисептические препараты. При этом наиболее эффективными являются многофункциональные и интеллектуальные покрытия, оказывающие комбинированное воздействие на микробные биопленки благодаря выраженным антиадгезивным и биоцидным свойствам. Отмечен дефицит исследований по применению многофункциональных покрытий в травматологии и ортопедии. В настоящее время отсутствуют публикации, посвященные использованию антибактериальных покрытий при лечении огнестрельных переломов и их последствий.</p></sec><sec><title>Заключение</title><p>Заключение. Применение полимерных и многофункциональных антибактериальных покрытий, гидрогелей, оксидов серебра, йода и цинка демонстрируют высокую эффективность в профилактике инфекционных осложнений после проведения внутреннего остеосинтеза, и могут быть использованы в клинической практике при лечении пострадавших с огнестрельными переломами костей конечностей.</p></sec></abstract><trans-abstract xml:lang="en"><p>Introduction High risk of infectious complications in gunshot wounds remains a pressing issue in military medicine. Analysis of the structure of sanitary losses shows that limb injuries account for 55 % to 81.4 %, with about 35 % of them accompanied by bone fractures. Performing operations for the final stabilization of these fractures under the conditions of primary microbial contamination is associated with a high risk of infectious complications. However, the use of antibacterial coatings on internal implants significantly reduces the risk of such complications.</p><p>The purpose of the work, based on the analysis of Russian and foreign literary sources, is to determine the effectiveness of using antibacterial coatings on titanium implants for gunshot fractures.</p><p>Materials and methods The search for scientific publications was carried out in the search engines eLibrary, PubMed and Connected Papers using the keywords: antibacterial coatings, gunshot fractures, implant-associated infection, internal osteosynthesis, infectious complications, antibacterial coating, gunshot fractures, infectious complications, peri-implant infection. The sources were selected based on the hypothesis of the possibility of using antibacterial coatings in clinical practice. The search depth was from 2009 to 2025.</p><p>Results and discussion The existing systems for delivering antibacterial drugs to the surgical intervention area demonstrate high clinical efficacy in the prevention of peri-implant infection. To date, the most studied agents for creating coatings are metal ions, polymers, as well as composites containing antibacterial / antiseptic drugs. The most effective are multifunctional and intelligent coatings that have a combined effect on microbial biofilms due to their pronounced anti-adhesive and biocidal properties. There is a shortage of research on the use of multifunctional coatings in traumatological and orthopedic practice. There are no publications in the world literature devoted to the use of antibacterial coatings in the treatment of gunshot fractures and their consequences.</p><p>Conclusion The use of polymer and multifunctional antibacterial coatings, hydrogels, as well as oxides of silver, iodine and zinc demonstrate high efficiency in the prevention of infectious complications after internal osteosynthesis, and, in our opinion, can be considered for use in clinical practice in the treatment of gunshot fractures of limb bones.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>антибактериальное покрытие</kwd><kwd>внутренний остеосинтез</kwd><kwd>огнестрельные переломы</kwd><kwd>инфекционные осложнения</kwd><kwd>периимплантная инфекция</kwd></kwd-group><kwd-group xml:lang="en"><kwd>antibacterial coating</kwd><kwd>internal osteosynthesis</kwd><kwd>gunshot fractures</kwd><kwd>infectious complications</kwd><kwd>peri-implant infection</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Оприщенко А.А., Штутин А.А., Коктышев И.В.
