Артродез голеностопного сустава, хирургическое вмешательство при последней стадии артроза и сложных патологиях голеностопного сустава, является альтернативой ампутации, сохраняя конечность.Цель работы представление клинического опыта использования аппарата Илизарова для достижения стабильного, безболезненного сращения голеностопного сустава у пациентов с различными сложными патологиями голеностопного сустава.Материалы и методы. Проведено ретроспективное исследование с участием 27 пациентов, которым выполнен артродез голеностопного сустава с использованием аппарата Илизарова в период с 2014 по 2024 гг. Клиническую и рентгенологическую оценку проводили по балльной системе ASAMI. Проанализированы хирургические методы, демографические данные пациентов и результаты.Результаты. У всех 27 пациентов произошло успешное сращение костей. Результаты ASAMI по костному сращению оценены как отличные у 23 пациентов и хорошие у 4. Функциональные результаты оценены как хорошие у 22 пациентов и удовлетворительные у 5. При спицевом инфицировании успешно применяли антибиотики. Метод Илизарова продемонстрировал превосходные результаты в достижении стабильности и безболезненности голеностопных суставов даже в случаях тяжелого остеомиелита и разрушениях с деформациями.Обсуждение. Аппарат Илизарова обеспечивает минимально инвазивный, универсальный подход к  сложным патологиям голеностопного сустава, позволяя осуществлять динамическую осевую компрессию, раннюю нагрузку и коррекцию деформации. Несмотря на такие ограничения, как высокая стоимость и требования к квалификации хирурга, процент успешных операций превосходит результаты использования методов внутренней фиксации.Заключение. Аппарат Илизарова является золотым стандартом для артродеза голеностопного сустава, обеспечивая стабильное сращение и устраняя сопутствующие заболевания, такие как остеомиелит и разная длина конечностей, имеет высокую степень удовлетворенности пациентов и функционального восстановления.</p></abstract><trans-abstract xml:lang="en"><p>Introduction Ankle arthrodesis is a surgical procedure for end-stage arthritis and complex ankle pathologies, offering a limb-salvaging alternative to amputation. This study aims to present clinical experience with the Ilizarov apparatus in achieving stable, painless ankle fusion in patients with varied complex ankle pathologies.Materials and Methods A retrospective study was conducted involving 27 patients who underwent ankle arthrodesis using the Ilizarov fixator between 2014 and 2024. Clinical and radiological evaluations were performed using the ASAMI scoring system. Surgical techniques, patient demographics, and outcomes were analyzed.Results All 27 patients achieved successful bone union. ASAMI Bone results were rated excellent in 23 and good in 4. Functional outcomes were rated as good in 22 patients and fair in 5. Pin tract infections were effectively managed with antibiotics. The Ilizarov technique demonstrated superior results in achieving stable, pain-free ankles, even in cases with severe osteomyelitis and destroyed ankles with deformities.Discussion The Ilizarov apparatus provides a minimally invasive, versatile approach for complex ankle pathologies, enabling dynamic axial compression, early weight-bearing, and deformity correction.
