В настоящее время частота травм по всему миру остается высокой и составляет при глобальной оценке около 6763 случаев на 100000 населения (95 % доверительный интервал 6412–7147). Травмы конечностей относятся к наиболее частым повреждениям отдельных анатомических областей в структуре множественной или сочетанной травмы и наблюдаются, по разным данным, у 40–85,2 % пациентов. Оценка эффективности разных вариантов остеосинтеза и разработка алгоритмов лечения пациентов с переломами голени и множественной (сочетанной) травмой является крайне актуальной для современной травматологии и ортопедии.</p><p>Цель работы — определить по данным литературы степень применения технологии временного остеосинтеза костей голени при лечении пострадавших с множественными и сочетанными травмами.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Поиск источников осуществлен за 2008–2023 гг. в базе данных PubMed и  электронных библиотеках eLibrary.ru, КиберЛенинка по поисковым словам и словосочетаниям: травмы голени, остеосинтез костей нижних конечностей, множественные травмы, сочетанные травмы; leg injuries, osteosynthesis of lower extremity bones, multiple injuries, combined injuries, temporary osteosynthesis of the shin bones.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. В настоящее время наиболее распространенным является дифференцированный подход к лечению пациентов с переломами при множественных и сочетанных травмах, когда выбор методики остеосинтеза основывается не только на тяжести травмы, но и тяжести состояния пациента. В то время как у стабильных пациентов преимущественно выполняют окончательный внутренний остеосинтез, у пограничных и тяжелых пациентов применяют тактику «контроля повреждений», — первичный временный внешний остеосинтез с последующим этапным хирургическим вмешательством. В данный момент среди профессионального сообщества нет общепризнанной тактики применения ранней мобилизации переломов длинных трубчатых костей как элемента противошокового мероприятия у пациента с политравмой.</p></sec><sec><title>Заключение</title><p>Заключение. Отдельные вопросы использования остеосинтеза при переломах костей голени у некоторых категорий пациентов, включая оптимальное время перехода к окончательному внутреннему остеосинтезу, возможность использования внеочагового остеосинтеза в качестве метода окончательной фиксации, а также оптимальная конфигурация и компоновка используемых аппаратов остаются открытыми. Важным ограничением является недостаток качественных рандомизированных контролируемых исследований в данной области.</p></sec></abstract><trans-abstract xml:lang="en"><p>Background The incidence of injury worldwide remains high, with a global estimate of  6763  cases per  00,000  population (95 % confidence interval 6412–7147). Trauma to the limbs is a common injury to an individual anatomical area during multiple or combined trauma that accounts for 40 % to 85.2 % of cases. Assessment of the effectiveness of different fixation options and development of treatment algorithms are essential for patients with tibial fractures and multiple (combined) injuries.</p><p>The objective was to determine how often temporary tibia fixation is applied for patients with multiple and combined injuries.</p><p>Material and methods The original literature search was conducted on key resources including Scientific Electronic Library (www.elibrary.ru), the National Library of Medicine (www.pubmed.org), CyberLeninka between 2008 and 2023 using search words and phrases: tibial injuries, osteosynthesis of lower limbs, multiple injuries, combined injuries, temporary osteosynthesis of the tibial bones.</p><p>Results and discussion A differentiated approach to the repair of bone fractures resulting from multiple and combined injuries is mostly common with the choice of fixation technique depending on the severity of injury and the severity of the patient’s condition. The definitive internal bone fixation is normally used for  stable patients, “damage control” strategy is secured for borderline and severe cases using primary temporary external fixation followed by staged surgical intervention. There is no generally accepted strategy for the use of early mobilization of long bone fractures as a component of anti-shock measures in a polytrauma patient.</p><p>Conclusion Certain issues remain unresolved, including the use of osteosynthesis for tibial fractures in some cohorts of patients, the optimal time of transition to definitive internal fixation, the possibility of using extrafocal osteosynthesis as a definitive treatment, the optimal configuration and assemblies to be employed.
