В попытке уменьшения риска развития гнойных осложнений артропластики тазобедренного и коленного суставов исследователи изучают возможности влияния на факторы риска в периоперационном периоде. Цель. Систематизация информации о потенциально модифицируемых факторах риска гнойных осложнений эндопротезирования тазобедренного и коленного суставов и возможностях влияния на них. Материалы и методы. Поиск литературных источников проводился в открытых электронных базах данных научной литературы PubMed, eLIBRARY, Scopus, Dimensions. Глубина поиска — 30 лет. Результаты. В обзоре проанализированы потенциально модифицируемые факторы риска и возможности влияния на них в периоперационном периоде. Часто пациенты с показаниями для эндопротезирования крупных суставов имеют коморбидную патологию, которую необходимо компенсировать на этапе подготовки к хирургическому вмешательству и в послеоперационном периоде. В том числе это может быть патология суставов: олиго-, полиостеоартроз, артропластика других суставов, септический артрит или перипротезная инфекция в анамнезе. Соматические заболевания нередко связаны с отклонением от нормы определенных лабораторных показателей. Все эти факторы риска не могут быть полностью устранены и расцениваются как неблагоприятный фон для проведения эндопротезирования тазобедренного и коленного суставов. Дискуссия. Современный уровень информации о возможных рисках развития гнойных осложнений артропластики тазобедренного и коленного суставов не всегда позволяет повлиять на снижение частоты возникновения инфекционных осложнений. Сохраняется неопределенность в отношении некоторых предикторов инфекции области хирургического вмешательства, перипротезной инфекции. Не всегда имеется однозначная причинно-следственная связь между потенциально неблагоприятным фоном пациента и негативным результатом операции, что требует дальнейшего углублённого изучения данной проблемы.</p></abstract><trans-abstract xml:lang="en"><p>The objective of the review was to systematize information on potentially modifiable risk factors for infectious complications following total hip and knee arthroplasty and the possibilities to control them. Material and methods For a comprehensive search, PubMed, eLIBRARY, Scopus, Dimensions were used. The search depth was 30 years. Results The review reports potentially modifiable risk factors and the possibility to control them in the perioperative period. Patients undergoing total joint replacements often suffer comorbid conditions that must be addressed preoperatively and postoperatively. Comorbidities can be associated with such joint pathologies as oligo-, polyosteoarthrosis, arthroplasty of other joints, septic arthritis or with a history of periprosthetic joint infection. Somatic disorders can be associated with abnormal laboratory findings. All these risk factors cannot be eliminated completely and are detrimental for hip and knee arthroplasty. Discussion The current level of information on the risks of infectious complications following total hip and knee arthroplasty may be insufficient to reduce the spread of an infectious agent. There is controversy regarding some predictors of surgical site infection and periprosthetic joint infection. There may be equivocal cause-effect relationships between the patient's potentially unfavorable features and the adverse outcome, which requires further in-depth study of this problem. </p></trans-abstract></article-meta></front><back><ref-list><title>References</title></ref-list><fn-group><fn fn-type="conflict"><p>The authors declare that there are no conflicts of interest present.</p></fn></fn-group></back></article>
