Цель – поиск, изучение и анализ литературных источников, определяющих место деформаций стоп в патогенезе синдрома диабетической стопы и посвященных роли методов ортопедической коррекции в комплексе лечения данного позднего осложнения сахарного диабета. Материалы и методы. Поиск литературы проводился по текстовым базам данных медицинских публикаций PubMed, CyberLeninka, Google Scholar, Scopus, Medline, eLIBRARY среди статей на английском и русском языках. Поиск осуществлен по следующим терминам: diabetes mellitus, foot deformities, diabetic foot syndrome, orthopedic treatment, osteotomy, metatarsophalangeal arthroplasty, аchilles tendon lengthening. Из исследования исключены литературные источники, посвященные методам ортопедической коррекции деформаций стоп изолированно от синдрома диабетической стопы, а также литературные источники, посвященные проявлениям синдрома диабетической стопы без упоминания ортопедических методов лечения (хирургия гнойных осложнений, сосудистая хирургия, сахароснижающая терапия). Результаты. Изученная литература позволяет высоко оценить роль ортопедического лечения у пациентов с синдромом диабетической стопы. Существуют два основных направления: консервативное лечение и хирургическое. Так, эффективность консервативных методов давно подтверждена в работах с высокой степенью доказательности. Хирургическая коррекция проявлений СДС за счет ортопедических операций в публикациях такого уровня освещена не так подробно. Заключение. Обзор литературы посвящен актуальной проблеме на стыке различных специальностей в медицине. Приведен анализ результатов лечения по данным отечественных и зарубежных авторов. Кроме того, изучены «пробелы» в представленной литературе, сформированы задачи и цели для последующих исследований в данном направлении.
The objective was to review the orthopaedic literature on treatments of diabetic foot disease.
Material and methods
PubMed, CyberLeninka, Google Scholar, Scopus, Medline, eLIBRARY were searched. Studies in English and in Russian that reported foot deformities in the pathogenesis of diabetic foot disease and orthopaedic correction methods in the complex treatment of this late complication of diabetes mellitus were included for review. All study designs were considered. The search was produced using the following terms: diabetes mellitus, foot deformities, diabetic foot syndrome, orthopaedic treatment, osteotomy, metatarsophalangeal arthroplasty, achilles tendon lengthening. Literature sources reporting methods of orthopaedic correction of foot deformities in isolation from the diabetic foot syndrome and literary sources reporting manifestations of the diabetic foot syndrome without description orthopedic methods of treatment (surgery for purulent complications, vascular surgery, hypoglycemic therapy) were excluded from the study.
Results
The literature review allowed us to evaluate the role of orthopaedic treatment of foot deformities in patients with diabetic foot syndrome to be addressed either conservatively or surgically. The effectiveness of conservative methods has long been reported in studies with higher degree of evidence. Surgical correction of DFS through orthopaedic interventions has not been covered in such detail. Conclusion The literature review focused on the topical issue at the junction of various medical specialties. The results of treatment reported by Russian and foreign authors were analyzed. "Blind spots" in the literature were explored, goals for subsequent research delineated.
