Прогнозируется увеличение распространенности остеопороза среди лиц старше 50 лет. Для улучшения диагностики данного заболевания было инициировано исследование по оценке эффективности клинического применения асинхронной количественной компьютерной томографии (ККТ). Цель. Анализ результатов внедрения асинхронной ККТ с учетом факторов риска, включенных в инструмент FRAX, в одно из амбулаторных учреждений города Москвы. Материалы и методы. Проведен одномоментный анализ оценок минеральной плотности кости (МПК) у пациентов, направленных по показаниям на ККТ профильными врачами-клиницистами в рамках одной медицинской организации. ККТ-сканирование включало две области сканирования: поясничный отдел позвоночника и проксимальный отдел бедра. Сканирование проводилось на 64-срезовом компьютерном томографе Toshiba Aquilion, а измерение МПК осуществлялось с программы “QCT PRO”. Также проведен выборочный анализ 10-летнего риска возникновения основных остеопоротических переломов с применением инструмента FRAX, критериями исключения из которого было следующее: проведение антиостеопоротического лечения, недостоверные данные в анкете, наличие артефактов на изображениях в области проксимального отдела бедра. По результатам проведен анализ распределения пациентов по группам показаний инициации лечения в соответствии со стратегиями FRAX, FRAX с коррекцией на МПК шейки бедра, ККТ, FRAX совместно с ККТ. Результаты. За год исследования проведено ККТ-сканирование у 710 женщин, средний возраст (СКО) которых составил 67,3 (9,3) года. Диагноз «остеопороз» по результатам ККТ трех областей интереса был поставлен 418 пациентам (59 %), остеопения – 252 (35 %), а норма наблюдалась только у 40 (6 %). Анализ 10-летнего риска основных остеопоротических переломов по FRAX произведен выборочно 111 пациентам, у 15,2 % выявлен высокий риск, а с учетом коррекции на минеральную плотность шейки бедра – у 14,3 %. По данным ККТ высокий риск переломов выявлен у 46,4 % анкетированных пациентов, у которых может быть инициировано назначение лечения. Применение ККТ дополнительно к FRAX позволило оптимально выделить пациентов, которым показано назначение антирезорбтивного лечения, что составило 30,4 %. Заключение. По результатам ККТ-сканирования остеопороз выявлен у 59 % пациентов, при этом при определении 10-летней вероятности переломов с использованием инструмента FRAX выявляется потребность инициации лечения только у 15,2 %. Использование совместно инструмента FRAX и результатов ККТ позволяет оптимизировать долю пациентов, которым требуется антиостеопоротическое лечение до 30,4 %. Результаты данного исследования могут быть использованы при разработке системы поддержки принятия врачебных решений для ведения пациентов с подозрением на остеопороз.</p></abstract><trans-abstract xml:lang="en"><p>Introduction The prevalence of osteoporosis is predicted to increase among individuals over 50 years of age. Research was initiated to evaluate the effectiveness of the clinical use of asynchronous quantitative computed tomography (QCT) facilitating the diagnosis of the condition. Objective Review the results of asynchronous QCT introduced in an outpatient clinic in Moscow, with reference to the risk factors included in the FRAX tool. Material and methods Bone mineral density (BMD) was measured in patients referred for QCT by specialized clinicians employed at the same medical institution. The QCT scanning included two areas: the lumbar spine and the proximal femur and was produced with the Toshiba Aquilion 64 CT Scanner, and BMD measured using QCT PRO. A sampling analysis of the 10-year probability of a major osteop orotic fracture was produced using the FRAX tool with the exclusion criteria of anti-osteoporotic treatment, unreliable data in the questionnaire, artifacts in the images of the proximal femur. Based on the results, patients were assigned to groups by indications for initiating treatment in accordance with the FRAX strategies, FRAX corrected for BMD of the femoral neck, QCT, FRAX in conjunction with QCT. Results Within a year of the study, QCT scans were performed for 710 women with the mean age (MA) of 67.3 (9.3) years. Based on CT findings of three sites of interest patients were diagnosed with osteoporosis (n = 418, 59 %), osteopenia (n = 252, 35 %), and 40 (6 %) showed normal manifestations. The FRAX-based 10-year probability of major osteoporotic fractures was used selectively for 111 patients with a high risk detected in 15.2 %, and BMD of the femoral neck adjusted in 14.3 %. QCT findings revealed a high risk of fractures in 46.4 % of the surveyed patients who could receive treatment. The use of QCT in addition to FRAX allowed optimal identification of patients who had antiresorptive treatment indicated in 30.4 %. Conclusion QCT findings detected osteoporosis in 59 % of patients, while the FRAX-based estimates of 10-year fracture probabilities indicated to the need for initiation of treatment in 15.2 % only. Using the FRAX tool and QCT findings together allowed optimization in the proportion of patients who required anti-osteoporotic treatment up to 30.4 %. The results of the study can be used in the development of a clinical decision support system for management of patients with suspected osteoporosis.</p></trans-abstract></article-meta></front><back><ref-list><title>References</title></ref-list><fn-group><fn fn-type="conflict"><p>The authors declare that there are no conflicts of interest present.</p></fn></fn-group></back></article>
