Заболевания, связанные с поражением верхних или нижних двигательных нейронов, такие как спина бифида, миопатия и остаточный полиомиелит, часто вызывают серьезные деформации стопы и голеностопного сустава.</p><p>Цель лечения – превратить деформированную и ригидную стопу в опорную.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С 2001 по 2018 год пролечена методом Илизарова 51 неврологическая стопа. В 27 случаях это были последствия полиомиелита, патология 17 стоп развилась вследствие спина бифида, у остальных 6 пациентов была болезнь Шарко-Мари-Тута.</p><p>Результаты оценивались путем сравнения внешнего вида и функции до и после операции, описанных Dimeglio и модифицированных Dong Y.L. с соавторами. До операции отмечено 33 деформации средней степени тяжести и 28 тяжелых деформаций (средний дооперационный балл 6,3). Среднее время, необходимое для исправления деформации, составило 37,1 дня (диапазон 22–58 дней), а среднее время стабилизации – 67,2 дня (диапазон 45–98 дней). При последнем осмотре в среднем через 7,42 года (от 13 месяцев до 16 лет) клинические исходы и удовлетворенность пациентов считались приемлемыми. У 28 пациентов достигли отличного результата, у 21 – хорошего, две стопы были классифицированы как плохой результат. Около 86 % пациентов (44 пациента) были довольны своими результатами и ответили, что повторили бы процедуру. Осложнения включали инфекцию вокруг спиц в 15 %, дизестезию в 5 случаях, поверхностный некроз в 3 случаях. У одного пациента развилась контрактура коленного сустава. Двум пациентам была проведена повторная операция с V-образной остеотомией.</p></sec><sec><title>Вывод</title><p>Вывод. Метод Илизарова открывает широкие возможности в лечении тяжелых неврологических деформаций стопы. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Diseases related to the lesions of upper or lower motor neurons, such as spina bifida, myopathy, and residual polio, often cause severe deformities of the foot and ankle. The treatment goal is to convert a deformed and rigid foot into a plantigrade foot.</p></sec><sec><title>Matherial and method</title><p>Matherial and method. We treated 51 neurological feet from 2001 to 2018 with the Ilizarov method. They were polio outcomes in 27 cases, 17 feet were affected by spina bifida, 6 other patients had Charcot-Marie-Tooth disease.</p></sec><sec><title>Results</title><p>Results. Results were assessed by comparing appearance and function before and after surgery described by Dimeglio, and modified by Dong Y.L. et al. Before surgery, there were 33 moderate and 28 severe deformities (average preoperative scores, 6.3). The mean time required to correct a deformity was 37.1 days (range, 22–58 days) and the mean time for stabilization was 67.2 days (range, 45–98 days). At latest follow-up (mean 7.42 years, from 13 month to 16 years), clinical outcomes and patient satisfaction were considered acceptable. 28 patients achieved an excellent outcome and 21 patients had a good outcome, two feet were classified as having a poor result. Nearly 86 % of patients (44 patients) were satisfied with their outcomes and replied they would repeat their procedure. Complications include infection in 15 % of K-wire tracts, Dysesthesia in 5 cases, superficial necrosis in 3. One patient developed knee contracture. 2 patients were re-operated with V osteotomy.</p></sec><sec><title>Conclusions</title><p>Conclusions.
