На сегодняшний день не существует единого подхода к выбору тактики хирургического лечения остеоартрита первого плюснефалангового сустава (ОА I ПФС). С другой стороны, известно, что при наличии костно-хрящевых дефектов в тазобедренном, коленном и голеностопном суставах успешно применяется техника аутологичного индуцированного матрицей хондрогенеза. В связи с этим нами предложено применение данной техники в лечении пациентов с ОА I ПФС.</p></sec><sec><title>Цель</title><p>Цель. Изучить возможность хирургического лечения пациентов с ОА I ПФС с использованием хондропластики по технике аутологичного индуцированного матрицей хондрогенеза, продемонстрировать ближайшие и среднесрочные результаты выполненных операций.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. К настоящему времени хондропластика выполнена 15 пациентам (16 стоп) с ОА I ПФС. При обследовании определялся объем движений в I ПФС; состояние оценивалось по таким шкалам как ВАШ боли, AOFAS, FFI. Хондропластика I ПФС осуществлялась по технике индуцированного хондрогенеза с использованием коллагеновой матрицы. Результаты операций оценивались через 3, 6 и 12 месяцев.</p></sec><sec><title>Результаты</title><p>Результаты. Уже через 3 месяца после операции отмечено выраженное значимое увеличение объема движений и уменьшение боли в I ПФС. Через 6 и 12 месяцев наблюдалась умеренная положительная динамика.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты проведенных операций показали, что хондропластика I ПФС является эффективным методом хирургического лечения, позволяющим купировать боль и существенно улучшать качество жизни пациентов с ОА I ПФС. При этом необходимо изучение отдаленных результатов. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. To date, there is no unified approach to the choice of tactics in surgical treatment of osteoarthritis of the first metatarsophalangeal joint (OA of the first MTP joint). On the other hand, it is known that the technique of autologous matrix-induced chondrogenesis has been successfully used for osteochondral defects in the hip, knee and ankle joints. Therefore, we have proposed to use this technique in the treatment of patients with OA of the first MTP joint.</p></sec><sec><title>Purpose</title><p>Purpose. To study the possibility of surgical treatment of patients with OA of the first MTP joint with the use of chondroplasty utilizing the technique of autologous matrix-induced chondrogenesis, to demonstrate the immediate and mid-term results of such operations.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Chondroplasty was performed in 15 patients (16 feet) with OA of the first MTP joint. The examination determined the range of motion in of the first MTP joint; the condition was assessed according to such scales as VAS for pain, AOFAS, FFI. Chondroplasty in of the first MTP joint was performed with the technique of induced chondrogenesis using a collagen matrix. The results of the operations were evaluated after 3, 6 and 12 months.</p></sec><sec><title>Results</title><p>Results. As early as 3 months after the operation, there was a marked significant increase in the range of motion and a decrease in pain of the first MTP joint. Moderate positive dynamics were observed after 6 and 12 months.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of the operations showed that chondroplasty of the first MTP joint is an effective method of surgical treatment that provides pain relief and significantly improves the quality of life of patients with OA of the first MTP joint.
