В настоящее, время в литературе достаточно широко представлены сообщения о морфологических, биохимических и гистопатологических изменениях параспинальных мышц у больных с идиопатическим сколиозом. Однако исследованию морфологической картины паравертебральных мышц у больных с тяжелыми формами идиопатического сколиоза уделено мало внимания.</p></sec><sec><title>Цель</title><p>Цель. Исследовать влияние угла деформации в грудном отделе позвоночника на морфологическую картину паравертебральных мышц у больных с идиопатическим сколиозом тяжелой степени.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Анализ результатов обследования 21 пациента с идиопатическим сколиозом. Выборка больных была разделена на две группы в зависимости от величины дооперационной деформации в грудном отделе позвоночника: основная группа – больные с углом деформации ≥ 60º (11 больных); группа сравнения – больные с углом деформации меньше 60º (10 больных). В процессе оперативного вмешательства для морфологического анализа паравертебральных мышц осуществляли биопсию фрагментов мышц (multifidus muscle), взятых в проекции вершины деформации (основной дуги) на уровне Th6-Th10 с выпуклой стороны при выполнении доступа к задним отделам позвоночного столба в ходе хирургической коррекции кифосколиоза. Проводили гистологический анализ биоптатов мышц с помощью световой микроскопии.</p></sec><sec><title>Результаты</title><p>Результаты. У больных с идиопатическим сколиозом при наличии тяжелой деформации (≥ 60º) были отмечены выраженные в различной степени явления атрофии и дистрофии мышечных волокон. Встречались безъядерные участки и гомогенизированные фрагменты мышечных волокон. Соединительнотканные прослойки почти во всех препаратах были отечными и разволокненными с множеством фибробластов. Стенки сосудов артериального звена, как правило, были фиброзированы. Диапазон варьирования структурных изменений колебался от минимальных отклонений от нормы (угол деформации 60º) до деградации мышечной ткани как структуры с замещением волокон жировой и соединительной тканью (угол деформации 145º).</p></sec><sec><title>Заключение</title><p>Заключение. У больных с идиопатическим сколиозом с тяжелой деформацией грудного отдела позвоночника величина угла искривления значительно влияет на морфологическую картину параспинальных мышц. Полученные результаты необходимо учитывать при хирургической коррекции кифосколиоза, уделять больше внимания сосудистой и нейротропной терапии в послеоперационном периоде для консолидации раны и снижения количества раневых осложнений. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Morphological, biochemical and histopathological characteristics of paraspinal muscles have been well described in patients with idiopathic scoliosis. However, there is a paucity of literature on morphological picture of paraspinal muscles in patients with severe idiopathic scoliosis.</p></sec><sec><title>Objective</title><p>Objective. To explore how the magnitude of thoracic spine deformity affects morphological characteristics of paraspinal muscles in patients with severe idiopathic scoliosis.</p></sec><sec><title>Material and methods</title><p>Material and methods. A total 21 patients with idiopathic scoliosis were reviewed. The patients were assigned to two groups depending on magnitude of preoperative thoracic spine deformity. A major group consisted of patients with scoliosis of ≥ 60º (n = 11) and control group included patients with a curve less 60º (n=10). Biopsy samples were taken from muscle fragments (multifidus muscle) at the apex of the curve (major arc) at the Th6-Th10 level on the convex side while approaching to the posterior spinal structures during surgical correction of kyphoscoliosis using light microscope. The specimens were examined histologically with light microscope.</p></sec><sec><title>Results</title><p>Results. Patients with severe idiopathic scoliosis (≥ 60º) showed evident muscle fiber atrophy and dystrophy of different extent. There were nucleus free areas and homogenized muscle fragments observed with connective tissue layers being swollen and frayed with multiple fibroblasts seen in nearly all the samples. Arterial walls were normally fibrotized. Structural changes ranged from minimal with a curve of 60º to degrading muscles with the fibers replaced by fat and connective tissues with a curve angle of 145º.</p></sec><sec><title>Conclusion</title><p>Conclusion. Morphological characteristics of paraspinal muscles have been shown to be severely affected by the magnitude of thoracic spine deformity in patients with severe idiopathic scoliosis.
