Несмотря на то, что при лечении пациентов с синдромом запястного казана (СЗК) хирургический метод признан высокоэффективным, его результаты не у всех являются одинаково положительными. Это предопределено исходной выраженностью клинических проявлений заболевания, объективизированной данными инструментальных методов обследования.</p></sec><sec><title>Цель</title><p>Цель. Дать клиническую оценку результатам оперативного лечения при СЗК с использованием опросников и показателей электронейромиографии (ЭНМГ) в сопоставлении с исходной степенью выраженности заболевания.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Изучили данные 161 пациента, которым по поводу СЗК выполнили 189 операций открытой декомпрессии срединного нерва из мини-доступа. Все клинические наблюдения распределили на 3 группы – с легкой (первая), умеренной (вторая) и выраженной (третья) степенями СЗК. Эффективность лечения оценили через полтора, 3, 6 и 12 месяцев после операции.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне в целом положительной динамики во всех клинических наблюдениях отметили замедленное восстановление функциональных возможностей кисти у пациентов третьей группы, не достигающее значений нормы даже через 12 месяцев после операции. Вместе с этим, у пациентов третьей группы влияние операции на течение СЗК было благоприятным, что проявилось «скачкообразным» улучшением функции кисти через 1,5–3 месяца после операции.</p></sec><sec><title>Заключение</title><p>Заключение. Открытая декомпрессия срединного нерва, выполненная у пациентов с СЗК из мини-доступа, позволяет у большинства достигнуть существенного клинического и функционального улучшения. Однако наиболее благоприятные результаты достижимы только при легкой и умеренной выраженности СЗК.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Although surgical treatment of carpal tunnel syndrome (CTS) is known to be highly effective outcomes may not be equally satisfactory for the patients due to severity of clinical presentation and objectifying assessment of the condition.</p></sec><sec><title>Purpose</title><p>Purpose. Provide clinical evaluation of outcomes of surgical treatment of CTS using questionnaires and electroneuromyography (ENMG) findings depending on baseline severity of the condition.</p></sec><sec><title>Material and methods</title><p>Material and methods. The review included 161 patients who underwent 189 operations of open decompression of the median nerve using mini-access. The patients were assigned to three groups with mild (Group I), moderate (Group II) and severe (Group III) CTS. Evaluations were produced at 6 weeks, 3, 6 and 12 months following the surgery.</p></sec><sec><title>Results</title><p>Results. The majority of patients showed positive dynamics, and delayed recovery of the wrist function was noted in Group III at a 12-month follow-up. Patients of Group III exhibited spasmodic improvement of the wrist function at 6-week-to-3-month follow-up.</p></sec><sec><title>Conclusion</title><p>Conclusion. Open decompression of the median nerve performed for patients with CTS using mini-approach facilitated substantial clinical and functional improvement in most cases.
