Цель исследования. Выявление особенностей переломов пяточной кости, сравнение результатов лечения различными методами и их эффективности при различных типах переломов, определение перспектив развития способов оперативного лечения. Материалы и методы. Исследованы результаты оперативного лечения переломов пяточной кости у пациентов, пролеченных за 3 года в травматологическом отделении. Результаты. Выявлены преимущества и недостатки всех использованных методик оперативного лечения. Составлен алгоритм оказания помощи пациентам с переломами пяточной кости и перспективы развития способов оперативного лечения. Заключение. Консервативное лечение показано при лечении переломов типа 73 А без смещения и остеосинтез винтами отрывных переломов со смещением; при переломах типа 73 В эффективна закрытая репозиция по Essex-Lopresti с фиксацией спицами либо винтами; при переломах 73 С - остеосинтез пластиной с костной аутопластикой; при открытых переломах 73 С методом выбора является способ по Essex-Lopresti с фиксацией спицами; ЧКДОС по Илизарову показан при инфицированных переломах, открытых переломах в сочетании с открытым переломом голени с той же стороны.
Purpose. To reveal the features of calcaneal fractures, to compare the results of treatment using different techniques and their effectiveness for the fractures of various types, to determine the prospects of the development of surgical treatment techniques. Materials and Methods. The results of surgical treatment of calcaneal fractures studied in patients treated in the traumatological department for three years. Results. The advantages and disadvantages of all the used surgical treatment techniques revealed. The algorithm made up to render care for patients with calcaneal fractures, as well as the prospects of surgical treatment techniques considered. Conclusion. Conservative treatment indicated for type 73A fractures without displacement, and screw osteosynthesis for avulsion fractures with displacement; closed reposition according to Essex-Lopresti with wire or screw fixation is effective for type 73B fractures; plate osteosynthesis and osteoplasty – for type 73C fractures; Essex-Lopresti technique with wire fixation is a method of choice for type 73C open fractures; transosseous compression-distraction osteosynthesis according to Ilizarov indicated for infected fractures, open fractures combined with ipsilateral open fracture of the leg.
