Переломы таранной и пяточной костей в структуре скелетной травмы регистрируются с частотой около 2 %, а среди переломов костей стопы повреждения заднего отдела преобладают – до 70 % случаев. Открытые переломы костей заднего отдела стопы диагностируются в 30 % случаев. Более половины таких повреждений относятся к III типу по классификации R.B. Gustilo, J.T. Anderson (1976). Ранение задней большеберцовой артерии наблюдается у 6,4 % пациентов. Частота ампутаций конечностей после костно-сосудистой травмы вследствие развития гангрены и размозжения мягких тканей составляет 3–60 %.</p></sec><sec><title>Цель</title><p>Цель. Продемонстрировать редкий клинический случай частичной травматической ампутации заднего отдела правой стопы при сочетании открытых переломов пяточной и таранной костей с повреждением задней большеберцовой артерии и положительным результатом лечения.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Клинический случай лечения пациента 36 лет с частичной травматической ампутацией заднего отдела стопы и повреждением задней большеберцовой артерии.</p></sec><sec><title>Результаты</title><p>Результаты. Достигнут положительный результат лечения в виде восстановления анатомии и функции правой стопы.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. В настоящее время отсутствуют клинические рекомендации и единая классификация костно-сосудистых повреждений, а также универсальный метод лечения такой сочетанной травмы – открытого перелома костей и ранения сосудов. Выбор метода лечения, диагностика костно-сосудистых повреждений проводится индивидуально, в зависимости от внешних и внутренних факторов, влияющих на эти повреждения. В данном случае использован ангиотравматологический подход к лечению пострадавшего с ранением заднего отдела правой стопы, состоящий из хирургического пособия в виде ранней первичной хирургической обработки костно-сосудистого повреждения, точной репозиции и прочной фиксации наиболее щадящим методом остеосинтеза отломков пяточной кости, восстановления задней большеберцовой артерии, комплекса медикаментозной терапии, направленной на улучшение реологии крови, снижения ее свертываемости, устранения ишемизации тканей, полноценного регионарного обезболивания.</p></sec><sec><title>Заключение</title><p>Заключение. Совокупность факторов и использование ангиотравматологического подхода с применением органосберегающей тактики с привлечением мультидисциплинарной бригады врачей травматологов-ортопедов, ангиохирургов, анестезиологов-реаниматологов позволили сохранить стопу пациента, восстановить ее функцию и опороспособность.</p></sec></abstract><trans-abstract xml:lang="en"><p>Introduction Talus and calcaneus fractures account for about 2 % of skeletal injury and 70 % of the foot fractures. Open fractures of the hindfoot are diagnosed in 30 %. More than half of the injuries are graded as type III according to classification of R.B. Gustilo, J.T. Anderson (1976). Injury to the posterior tibial artery is observed in 6.4 % of patients. Amputations resulting from an osseous-vascular injury and gangrene and crushed soft tissues are observed in 3-60 %.</p><p>The objective was to demonstrate a rare clinical case of traumatic partial hindfoot amputation on the right side associated with open calcaneus and talus fracture and injury to the posterior tibial artery and resulted in a good outcome.</p><p>Material and methods A clinical case of a 36-year-old patient who suffered a traumatic partial hindfoot amputation and injury to the posterior tibial artery.</p><p>Results Restoration of the anatomy and the function of the right foot was achieved in the patient.</p><p>Discussion There are no clinical guidelines and a unified classification for bone and vascular injuries and no universal method for the treatment of a combined injury including an open fracture of bones and injury to blood vessels. The choice of treatment modality, diagnosis of osteovascular injuries are produced on an individual basis depending on the external and internal factors affecting the injuries. An angiotraumatological approach used to treat the patient with the hind foot injury on the right included early primary surgical treatment of the osteovascular injury, accurate reduction and stable fixation using a sparing technique for the calcaneus, repair of the posterior tibial artery, complex drug therapy to improve blood rheology, reduce coagulability, address tissue ischemia and provide adequate regional anesthesia.</p><p>Conclusion The combination of factors and use of an angiotraumatological approach employing organ sparing strategy and a multidisciplinary team of orthopaedic and trauma surgeons, angiosurgeons, anesthesiologists-resuscitators facilitated foot salvage, improved function and supportability.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>сочетанная костно-сосудистая травма</kwd><kwd>открытый перелом пяточной кости</kwd><kwd>повреждение задней большеберцовой артерии</kwd></kwd-group><kwd-group xml:lang="en"><kwd>combined osteovascular injury</kwd><kwd>open fracture of the calcaneus</kwd><kwd>injury to the posterior tibial artery</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Jackson JB 3rd, Jacobson L, Banerjee R, Nickisch F.
